FirstLight Health Systems Awarded Health Care Homes Act
Health care providers are charged with many goals. One of the latest initiatives is developing a new model of delivering primary care across populations of patients with chronic health conditions through the development of health care homes. A “health care home” also called a medical home, is an approach to primary care transformation in which primary care providers, families and patients work in partnership to improve health outcomes and quality of life for individuals with chronic health conditions and disabilities.
FirstLight Health System was one of only five clinics in the state to receive a grant designed to establish a health care home. The goal of transforming primary care is to improve access to medical services, provide coordination, and increase patient experience. It thereby improves the quality of care patients receive and, and most importantly, their health. The focus remains on what the patient needs, reducing errors, giving a higher priority to preventive services, and improving the coordination and integration of care. This groundbreaking approach to health care is the beginning of a new and better process of care delivery for our communities.
In this model the health system continually works to improve a number of care principles. Some of the principles are patient focused care, multi-functional care planning, care management, and care coordination. In this model all the staff, as well as the patients, need to be engaged and supportive. It will take everybody’s commitment to make sustainable progress. Ultimately, the results are higher quality of care and reduced health care utilization.
